Abstract
Two important parts of reliability analysis should be to identify, investigate, and quantify accident sequences that lead to blackouts and to have appropriate levels of quality control, data collection, and system understanding. The output of this approach should inform - not dictate - reliability standards and requirements, which should be enforced with penalties that will make rational organizations in the industry follow the rules.
